Peripheral Arteriovenous Malformations (AVMs)

What are Peripheral Arteriovenous Malformations (AVM)

A peripheral arteriovenous malformation (AVM) is a type of AVM that occurs outside of the brain or spinal cord, usually in the arms, legs, hands, or feet. It is a rare condition where there is an abnormal, direct connection between arteries and veins, bypassing the capillary system. This can cause symptoms such as pain, swelling, discoloration, or a throbbing sensation. Peripheral AVMs can be present from birth or may develop later in life, and their cause is not well understood. Treatment may involve surgical removal, embolization, or a combination of both, depending on the location and severity of the AVM.

Peripheral arteriovenous malformations (AVMs) can be dangerous depending on their size, location, and the severity of symptoms they cause. Some people with small or asymptomatic AVMs may not experience any serious health issues, while others with larger or symptomatic AVMs may be at risk of complications such as bleeding, ulceration, infection, or limb ischemia. In rare cases, untreated peripheral AVMs can lead to limb loss or even death. It is important to seek medical attention if you suspect you have a peripheral AVM or are experiencing any symptoms related to it.

Different types of peripheral arteriovenous malformations (AVMs):

There are several types of peripheral arteriovenous malformations (AVMs), including:

  • Simple peripheral AVMs: These are the most common type of AVMs, and they consist of a single abnormal connection between an artery and vein.
  • Complex peripheral AVMs: These are larger and more complicated AVMs that may involve multiple abnormal connections between arteries, veins, and capillaries, and can lead to serious complications.
  • Syndromic peripheral AVMs: These are AVMs that are associated with genetic syndromes, such as hereditary hemorrhagic telangiectasia (HHT), which can cause multiple AVMs throughout the body.
  • Acquired peripheral AVMs: These are AVMs that develop because of trauma, injury, or surgery, and can occur anywhere in the body.
  • Congenital peripheral AVMs: These are AVMs that are present at birth and can range from small and asymptomatic to large and life-threatening.
AVM treatment

Treatment for peripheral arteriovenous malformations (AVMs):

The treatment of arteriovenous malformations (AVMs) depends on the size, location, and severity of the malformation. The goal of treatment is to prevent complications, such as bleeding, and to improve symptoms.

Treatment options for AVMs include:

  • Embolization: This procedure involves injecting a substance into the blood vessels to block the abnormal connections and decrease blood flow to the AVM.
  • Surgery: Surgical removal of the AVM may be necessary if it is causing significant symptoms or if there is a risk of bleeding.
  • Radiation therapy: This treatment uses high-energy radiation to destroy the abnormal blood vessels in the AVM.
  • Observation: If the AVM is small and asymptomatic, it may be monitored over time to ensure it is not growing or causing any problems.
AVM overview
The choice of treatment depends on the individual patient’s situation and the expertise of your vascular surgeon. A team of healthcare professionals, including neurosurgeons, interventional radiologists, and radiation oncologists, may be involved in the management of AVMs.
