Had the great pleasure to use the #Auryon laser atherectomy device for the first time with Dr Terrence Teo at Mount Elizabeth Novena CVL last week on a 68 year-old suffering from left sided short distance intermittent claudication affecting her #qol. The aim was to get a lasting effect, which plain angioplasty alone does not achieve.
She had a long left #SFA occlusion from near the origin to P1. We performed a SAFARI technique to cross mostly true lumen all the way. The lesion proximally was quite soft but distally very hard which required a retrograde crossing. With 2 passes of the #Auryon 2.0mm laser catheter we managed to create a decent channel and at the same time aspirate any clots from the proximal part of the vessel. After using high pressure non compliant balloon angioplasty using the #OrbusNeich #Jade balloon platform, we successfully lined the artery with sirolimus eluting balloons and obtained fast flow and excellent vessel expansion with minimal recoil, on the final angiography runs. She was walking the next day without any pain.
